What You’ll Do:

• Deliver patient-centered care in nursing home settings.
• Conduct assessments, diagnose, and create treatment plans.
• Collaborate with physicians and care teams.
• Educate patients and families.
• Maintain accurate documentation and comply with regulations.

What You Need:

• MSN from an accredited program.
• Board Certified Nurse Practitioner.
• Active NY License.
• Strong clinical, communication, and team skills.
• New grads welcome!
